If you are contacted by law enforcement politely decline the opportunity to speak with them.
If the person receiving the unusual photo was a minor, yes very big trouble could appear.
If the person targeted was a well connected adult, trouble might be possible.

No one can ever say what law enforcement will do.

The best thing you can do is shut yer yap, wait, and lawyer up.
