Filing an appeal

I filed an appeal to a summary judgment entered. I was informed by the clerk's office that the appeal has not bene moved forward becausei Ma missing information. Can anyone tell me what documents exactly are needed. I submitted an NOA and a statement of why I was appealing the decision.
