Roomate Evicting A Roommate On My Lease

Hi I'm from Salt Lake City, Utah, and I wanted to know the best way to evict my mother who has stayed with me, rent-free for 2 years. I'm finally tired of taking care of everything, and she won't get a job to help pay utilities, like we'd verbally agreed. She was added to my new lease on 01/01/2015. I briefly talked to my landlord and she was quite understanding about the issue and agreed to drop her from the lease (I have been living here for approx. 5 years, and I've been a good tenant) since I'm the primary leasee. and we could write up a new lease with just myself on it.

Is that something that she has the power to just do? To just remove my mother from the lease? And who would be issuing the eviction notice? Myself, or my landlord?

If your landlord believes he or she can evict your mother, you're best advised to stand back and watch the fireworks.
