emergency guardianship

I am fighting stage four breast cancer and now have two brain tumors. I have lived with my mother since the onset of this disease. I have a 16 year old and a four year old with me also. I would like my mother and eldest son (age 21) to have guardianship if something unforeseen happens to me. The 16 year old's father does not have a problem with this and the four year old's father has had nothing to do with his daughter, not even child support. How can I make sure that my mother and son would get guardianship?

I do think you and Mom (and Dad of 16 year old if possible) need at least an initial consultation with an attorney. It will be worth the money.
