Disbility denial of continuance

My husband has been on LTD for almost 3 years with a condition that causes him to twitch and jerk. During that time he has also began to suffer from depression due to lack of work. The LTD carrier provided him notice that they are denying further payments of his claim based on a diagnosis of "conversion disorder". They were very selective in the doctor's notes they chose to include in their review. We have a limited time to appeal, but don't know how to start the process. I would appreciate any advice.
