Custody with No marriage

What is the easiest way to get custody (in writing) when there was no marriage.
And also get visitation, child support, etc, in writing.
Child's father is named on the birth certificate, and he has exercised all of his rights as a dad, so has laid claim as father, and is recognized as father. That's not the issue, so paternity won't be an issue.
But the visitation/child support, etc, is becoming an issue that is not getting worked out between the two parents, as amicable as it once was.
They are both married now to other people, and both have other kids with those spouses.
They (at this point) live in the same town, so joint physical/legal, would actually work at this point and time.
However it is something that needs to be court ordered and "legal"
Suggestions.

To have enforceable visitation and/or custody, he will have to go to court. It might be as simple as seeing a mediator and having the court endorse whatever agreement is decided on, but ultimately, no plan is enforceable absent the court's blessing.
