Credit life Insurance

My wife passed away last December. She had a personal loan with a certificate of deposit as collateral. The note had been rewritten twice at each maturity. When i went to close out her account, I was told that on the last renewal of the note there was no credit life.
When I finally got the paperwork, i found that the last maturity there was not a new note written, but the existing note had been extended. The monthly payment stayed the same as before. As part of each payment was for the credit life, should it not have extended along with the note?
