How can I report instance of corporate malfeasance? Who do I report to? Is there a government agency?

I'd be very careful reporting what you consider malfeasance.
If you believe the company is engaging in unsavory practices, its much easier to simply sever all ties and walk away.
If you must tell someone, you could start with the Board of Directors, "C" level executives, access the entity's internal policy about corporate ethics and see where one might start, or your state's attorney general.
