Adult child of evicted tenant seeking reference

I legally evicted a mother/boyfriend tenant in March. They were renters since 2004, had a 1 year lease that reverted to MTM after the first year. She had two children living with her, one 17 and one 20 at the time of eviction--the 20 year old was named on the eviction papers but didn't show up for court, his mother and boyfriend did. I received a rental reference form from the Housing Authority for the adult child asking a series of yes/no questions and whether or not he was on the lease. I really don't want to trash him, as I feel his mother was responsible for the rent, but how should I respond to this form?
 
Seems pretty easy to answer since these are yes or no questions. Was the 20 year old listed on the lease? If yes, answer so. Since he was considered an adult at the time of your lease (and was included in your eviction notice) he did have a legal obligation in regards to meeting your rental requirements.
