My husband and I live in Virginia (USA) and have been seperated for 2 years. I attended a 401k meeting today and if I do not wish my spouse to be the benegiciary, he must sign his consent. My husband has had a 401k plan for years. Is this a law for all 401k plans? I never signed anything concerning his plan. There is the chance that he put me as beneficiary but, what do I do if he forged my name or had someone else do it?
