Where can I find a template for a disposition proposal (Texas) for child support & custody modificat

I have filed a modification for child support and custody in Texas. My ex has contested everything and we recently went to meet with the judge for a status conference. She is requesting a disposition proposal for all issues and support. I've looked on txlawhelp.org and a couple of other websites but I am having trouble finding what exactly needs to be included. I have some legal background as I worked as a paralegal for 5 years, but we did not handle family law. I am pro se and really would like some advice or someone to lead me in the right direction to assist drafting this document. Thank you.
