Question about an NDA

I am a Freelance web developer working on UpWork. Recently found a client and had a phone meeting discussing the project they were very broad about the project not giving many details I was interested and proceeded with the project.

We have a meeting coming up to discuss in more detail but first he requested I sign an NDA. But's few days before the meeting he has asked me for my address to list on the NDA is this common practice or should I be careful?

He means we only deal with US law questions and you didn't mention a state you are in.
