Consumer Law, Warranties Permission to use photos, etc.

I am starting a business (in Pennsylvania) which is going to require me to obtain pictures from customers and make use of them in computer applications. Therefor, I will essentially be selling a product that contains these pictures. My question is, what type of legal form would I need customers to agree to/sign, in order to cover myself in the event they are providing photos that they have no right to use. Basically, something to state, "yes these are my photos and/or I have the legal right to use them"." Or whatever else would need to be said, in legal terms obviously. What I don't want, is for someone to provide photos, of say someone else, and that someone else to later be angry about it, and then try to take legal action against me. Any advice to point me in the right direction would be greatly appreciated. Thanks in advance.
