Name Change Procedure

My fiance is 30 years old. When he was around age 12 his divorced mother remarried, and the family moved from CA to WA. His mother and step-father began using the step-father's last name for the children when enrolling them in school, etc. My fiance's SS card, all forms of ID, bills, etc, have his stepfather's last name, although his step-father never legally adopted him. We have now moved to another state and my fiance needs to get a driver's license here, and he also needs to get a passport. However, the name on his birth certificate is different from all other forms of ID and proof of residence that he has, and his parents never had his named changed legally through adoption or otherwise.

Please advise on what he needs to do now to get his name changed legally. Since all his bills, SS card, ID, etc carry his stepfather's last name, it seems simplest to just have his name legally changed to that. How do we go about that and are there fees involved? We are now living in Pennsylvania.
