Insurance paid to wrong person

My dentist does not submit claims for secondary insurance, so when my wife's primary benefit ran out, she had to pay $412, which we then submitted to my insurance where she is also covered. They approved $399 of that amount, but then instead of sending the payment to us, they sent it to the dentist office. The form we used has a space for the dentist name and address if they payment is going to them, but you leave it blank (which we did) if the payment is to be sent to the patient. The dentist office says their policy is to apply it to our account which would give us a credit with them, and the insurance company won't send us payment until they recieve the payment back from the dentist. The insurance company ALSO said the reason they sent it to the dentist instead of to us, was because thats how the primary claim was done, despite the fact that we used their own form, filled out correctly, to file the claim.
