Starting a Business How to form a DBA under a LLC?

Hello,
I formed a LLC a few weeks ago. I would like to form a DBA for the proper naming of my business for marketing materials, etc.

I was filling the DBA form and had a few questions.

1.
business to be conducted as: sole practitioner, sole proprietor, general partnership, joint venture, etc
- I have a member run LLC and i am unsure what to circle here.

2.
NAME OF OWNERS:

should I put my LLC name here? or personal info?

You might want to consult a lawyer, or discuss your response with others involved in your venture.

What form are you filling out? If you got it online, post a link to it.

The state form 503 has a box to check for Limited Liability Company. You check that.

If you are filling out a county form that doesn't have a box for Limited Liability Company, you check "other" and write in Limited Liability Company, like on this one:

NAME OF OWNERS:

should I put my LLC name here? or personal info?

I don't see anything on either form that asks for "Name of Owners" so give us a look at what you're filling out.
