My question involves criminal records for the state of: Indiana

I am preparing to hand-deliver my petition to the Starke County DPA to petition for the expungement of my record. I was arrested and charged on December 12th, 2012 with a Class D Felony of Breaking and Entering and a Class B Misdemeanor of Criminal Mischief. I have prepared two documents thus far and this Friday will obtain the Indiana State Police record. I feel as though I may be missing parts of this procedure or even forms.

Listed below are two links to the two forms I have almost entirely prepared. This Friday this will be completely filled out in their entirety after I obtain information from the Indiana State Police.

I understand that there is an eight year waiting period after the conviction, but I am also aware that the prosecuting attorney can provide a letter of consent to remove the charges. I discussed this possibility with him last year by phone and he said that of this year, if three years had passed and I had not had another charge or conviction that he would agree to provide this letter of consent. I don't know how to proceed with the letter of consent, either, but according to his clerk I have to first file the petition.

P.S. I am ordering green paper for the pages required to be printed on green paper this Friday. My record has kept me from providing income for my family on many occasions. Your help will be met with great gratitude!
