My opinion?

First of all, identifying your former employer and posting your address is a terrible idea.

Beyond that, I'm not really sure what sort of response you're seeking or expecting.

Also, your post doesn't entirely make sense. You started by telling us that your former employer is "still paying [you]," but then you wrote that you're "not receiving anything" and that the employer is "not reporting anything." If you're "not receiving anything" and your former employer is "not reporting anything," why do you claim that the former employer is "still paying [you]"?

I'm also not sure what we're supposed to make of the documents you attached.
