Durable Power of Attorney

My cousin is in the hospital right now with Menengitis. He also has cancer and HIV. He was on SSI and they switched him to SS which took him off of medicaid. Anyways, now he has no insurance and we are trying to get him his Social Security card and a Certified Birth Certificate (which he needs for the program with the hospital). He has already agreed to sign the Durable Power of Attorney, but my question is, will it give me the right to go and sign for him to recieve these things? He isn't able to do these things because he's in the hospital. He lives with me and my parents in Fort Worth, TX.
I went online and printed up the form for Durable Power of Attorney, so my other question is, is it still binding if I get the two signatures of witnesses to his signing?
