Hello, my name is Lynn. Me and my boyfriend just recently had a child a year ago. When I apply for my daughters birth certificate . I put my boyfriend's name wrong. Now I have to contest the paternity affidavit. I just need to know how to fix it. Please.

There are many DANGEROUS, CRAZY, PERVS, PIMPS, and KILLERS in this world.

If you simply spelled his name incorrectly, a correction is made by contacting the Corrections Section of the ISDH Vital Records office at 317.233.2700 and ask the friendly service representative for instructions on correcting the misinformation.



However, if you mistakenly listed JOE BROWN as the putative father, but meant to list JIM JOHNSON as the father, that is corrected by contacting the Indiana State Dept. of Health (Indiana Putative Father Registry) at 317/233-7085 to request a form. The nice representative will instruct you (and/or HIM) accordingly.

If, however, the "mistake" was more serious, you might have to end up petitioning a court.
