Collection agenct & court

I lost my kidneys in 2003 while unemployed and un insured. Paid the county hospital and lost most of my savings. Now the dialysis unit I was using has sent their bill to a collection agency. We have been talking for 6 months. I offered $4000 to settle a $16000 debt. Last month I raised the offer to $9500. They won't accept my offer and want to place a lien on my home. Has anyone else on this site had a similar situation? What can I expect when this does go to court? Does the judge have discretion in setting the amount that he/she orders me to pay.
