Advanced Health Directive, No POA

Good Morning! I need to draw up an advanced health directive that I can have on file with my medical records and health providers. The problem is that I have no one I can designate as a health Power of Attorney. Really, I have NO ONE who can fulfill this role. Can I record my wishes in a document and make it available to my medical providers and on my phone? If I do will it supersede anything I have written down in the past like a new will would?
Thanks for all input.

If you know of no person you could name as your agent, isn't it obvious that you can't complete an advanced medical directive?

Your Ohio State Bar provides some useful information on the topic.

Alternatively you can seek information from your physician or the hospital or medical facility you patronize.

Most large hospitals or clinics have social workers trained in the medical directive process who'll be happy to discuss potential solutions for people who are orphaned (and stand alone) in this life.
