Corporate Law Adding a member to LLC

Hello all you wise people,
I need your help :)

I registered my company as a one-member LLC (New Jersey) a couple of years ago. Now I want to add my wife as a second member (something we should have done in the beginning), and assign 51% of ownership to her, 49% to me (to become a woman-operated business, which is what it is in reality anyway).

I contacted the state and someone emailed me that I should file a Certificate of Amendment to the Certificate of Formation.

I took a look at the certificate of formation, and all it mentions is:
- the name of the company
- the name and address of the initial registered office and agent
- the duration of the company's existence
- states that "this LLC has one or more members"

So what is the language/wording I should use to make the changes? What do I put in the amendment field?

Also, I just went and filed an early annual LLC report online and added my wife as a member there, for starters.
