Deny visitation

My daughter's father is required by court order to provide me with a letter from his doctor 2 weeks prior to all visitation. His deadline for this comming visitation has come and gone. What can i file with the court to notify them that he is not following the court order and to propperly deny the visitation? This case is in Florida.
 
The problem is that i live in TX, it would be easier if i were there. I need to know what kind of petition, request,or form of paperwork is needed to have the judge decide if this is reason enough to deny the visit. I have been held in contempt once for dening visitation and don't intend on doing that again.
 
If you have an Attorney work through him or her. If not contact court and ask. Until you know for sure do not deny visit. He is in contempt not you at present. Unless given the authority to deny vists if he fails to meet his end of bargain it is not wise to refuse visit
 
The judge stated in the court room that visitation could be denied for this reason but it is not stated in the final order. Thanks for your help.
