Qualified medical child support order

Hey i have a question hopfully some one can help with. I am trying to get my son on my health insurance and missed the enrollment period. I got a qualified medical child support order (qmcso). Now it seems pretty straight foward except this part. The part i had underlined. This is just a sample form to. And also it asks who the petitioner is and the respondent. Would it be the same as the child support case? Or am i automaticly the petitioner? I ask because originally she took me to court. So on the court paperwork she is the petitioner. I hope i explained this well enough i thank you for any help.

Generally.

If you bring the lawsuit you are referred to as petitioner.
So on the court paperwork she is the petitioner.

The opposing party is known as the respondent.

I suggest you do further research before proceeding.

Litigating a matter before any court is more than completing a form.
