You need to ask that question of your attorney, your employer's HR department (or benefits rep), or the WC Insurer.

This is your state's WC site. They probably can help you:

http://www.nh.gov/labor/workers-comp/

I've seen it take as little as a week to 10 days, and I've also been told of people waiting four to six weeks.

In the end, if you're approved, you'll receive all monies due and owed (eventually).

Has a settlement been ordered or even drafted and sent to the attorney representing your carrier? Settlements are not automatic and just a statement by a doctor that indicates you have some degree of permanent impairment on its own means nothing.

Call them back and ask for a case update. Ask them what their counter offer was.
