My 6 year olds biological father is ready to terminate his rights. He has never been around, in and out of jail, my son does see his grandparents and will continue to see them after rights are terminated (its not fair to my son or grandparents to take that away considering they are good people). I am married and have twin boys. My husband wants to adopt my son. Is there a form that we can get for him to sign his rights over to my husband without having to get an attorney? If so where can I find it, or get it from. My sons biological father is not on his birth certificate, but paternity was determined by the state.

Sorry, children aren't just signed off, get ownership transferred like one does a boat, a car, or even a home.
Children aren't property.
Parents don't own the children they've created, birthed, nurtured, and love.

If you seek to have your husband adopt this child, get ready to spend lots of money, wait a very long time, and endure a rigorous adoption process.

An internet forum can't do anything to help but point you in the direction of where this can start, and where you can begin to educate the others that desire to be involved.

Here you go:

Here you go for terminating parental rights:

Good luck.

Thank you, is there any quick way for him to sign his rights over, then we can pursue getting an attorney when the courts are involved?

You just don't transfer parental rights like you transfer ownership to a car.

No, there is no quick way that would mean anything.

See this form,

That's what he would sign, but it needs to be ordered by judge.

Nope, there is NOTHING quick or easy about what you wish to do.

See a lawyer, and you cna ask him or her more questions.
This will be an arduous exercise, and will take at least year.
