As a therapist, do I have to testify in court if I'm given a subpoena through certify mail by an attorney in Ohio - not signed by a judge or file stamped by court?

The rules of civil procedure vary from state to state.
In your state, according to the rules of civil procedure, service by certified mail is allowed.

I suggest you appear, otherwise you might be held in contempt.
If you believe you can't be compelled to testify because of your profession, I suggest you retain counsel to represent you and your point of view.
You and your attorney can appear for the express purpose of arguing why you can't be compelled to testify.
No one sould ever summarily decide they don't have to appear, becuase that often creates more havoc for them.
