relinquishing parental rights

Are the parents still married?

Were they ever married?

Has paternity been established?

Where can I obtain the legal documents to relinquish parental rights voluntarily?

You should hire a lawyer, but at a minimum, Texas allows "limited scope representation" by an attorney.

These are lawyers who will help you help yourself.

You hire a lawyer to offer advice, review forms, draft documents, or help you prepare yourself for a hearing/court proceeding.

The costs will vary, but its generally about 40-60% less than full representation.

You asked for forms, here's one.

But, it usually requires a bit more than completing a form.

I suggest you Google (relinquishing parental rights YOUR COUNTY,Texas).
