Does a Gift Warranty Deed remove tax liability from the grantor?

A friend's mother had a Gift Warranty Deed drawn with the thought of eliminating the property from her estate for tax liability or indebtedness if she became nursing home-bound. The language of the deed leaves all property records at taxing authorities in the mother's name which appears to indicate that the property is not out of her name for financial liabilities consideration.

Is my interpretation of this correct?
 
You should tell this potential tax evader that she desperately needs to retain a lawyer.
The taxman, and his pals, social insecurity and Medicare aren't falling for that. It's been tried many times.
