I should have answered this in full. The answer is - it depends. Many a times, the do have you go in on your own, but recently the trend is to have someone there to avoid having people tamper with the evidence...

The honor system typically only works when voluntary, e.g. A job application at a Fortune 500 company.There is also no need for a chain of custody and is merely for screening (although that too could be circumvented without the 'guarantor' nearby.)
 
As this is asked in the Criminal forum, I'll assume it's by order of probation/parole pre0sentencing, etc.

Not always. SOmetimes people stand behind you. Sometimes they are next to you, but this can cause "shy bladder." Sometimes there is a curved mirror in front of you, and the person watches from a room behind you, which seems to diminish shy bladder.

If you have shy bladder, tell whoever is supervising you. Sometimes they will let you take a blood test instead.
