Insurance Company writes lapsed policy

I have a limited pay insurance policy for medical bills. I have never used it until this last year. Now when a bill is submitted they send letter to me and billing medical entity that it either is not covered under policy or that the policy is lapsed. I have called several times in last two weeks and reminded them that policy is not lapsed and is still being deducted from my bank. The representative says they will refile and that they don't know what is going on. The trouble is when the next batch of bills comes in it is the same scene. Not covered or policy is lapsed. What can I do?
 
There is an administrative method where you can dispute their determination and it should be on the paperwork. Follow the procedure and send it CRRR.
