Tax evasion - fraud?

Jurisdiction/Place:

My former employer may have withheld information from the IRS regarding my bonus income. My question is three part.

A portion of my bonus income (minimum annual bonus) was in-directly paid to me in the form of health insurance premiums through out the course of my employment. More simply put, the employer paid premiums each month for the insurance, and as it was agreed to verbally on my hire date, he would then deduct those premiums form my year end bonus. Considering I was never taxed, one could assume that he claimed the tax deduction on his returns.

1. Should there have been payroll deductions applied to this portion of income?

2. Assuming that the employer did take such a deduction on his returns has any fraud been committed?

3. Do I have any obligation to notify the IRS ?
